BACB Monthly Verification Form: Multiple Supervisors at One Organization
If you are completing supervised fieldwork with more than one supervisor at the same organization, the BACB form choice matters. The current 2027 Monthly Fieldwork Verification Form: Multiple Supervisors at One Organization is designed for a coordinated organization-level arrangement that fits the applicable BCBA or BCaBA fieldwork requirements. It is not simply a longer version of the individual-supervisor form, and it is not the same document as the Final Fieldwork Verification Form.
The most important practical rule is to identify the organization and the responsible supervisor before you start entering hours. The current BACB FAQ says the responsible supervisor is the person listed on and signing the monthly multiple-supervisor form. The final form has a different job: it lists everyone from the organization who provided supervision during the fieldwork period, while the responsible supervisor signs the final attestation.

Which monthly form should you use?
Start with the supervision arrangement, not the number of signatures you expect to collect.
- Multiple supervisors at one organization: use the current organization form when the trainee’s fieldwork is coordinated within one organization and more than one supervisor provides supervision under the applicable requirements.
- One supervisor: use the Individual Monthly Fieldwork Verification Form when the month and fieldwork arrangement are documented with one supervisor.
- Separate organizations or settings: do not combine them just because the same trainee has one overall fieldwork plan. Review the applicable handbook and coordinated-fieldwork rules to determine which form belongs to each arrangement.
The BACB’s current fieldwork FAQ gives a useful example: a trainee has two supervisors at an ABA clinic and another supervisor at a university-affiliated setting. The clinic arrangement uses the organization multiple-supervisor form, while the other setting has its own documentation arrangement. The general lesson is to document each organization or coordinated setting according to the form and requirements that actually govern that work.
Form versions also matter. A trainee completing hours under an earlier set of fieldwork requirements may need the form associated with that period. The 2027 form is current for the 2027 requirements, but a year label alone does not decide which form applies to every historical month. Check the relevant BCBA or BCaBA Handbook and transition guidance before replacing a form that belongs to an earlier period.
What is the responsible supervisor’s role?
The responsible supervisor coordinates the supervision arrangement and is identified in the supervision contract. The role is more than being the person with the most meetings or the highest title. The responsible supervisor needs to be able to attest that the fieldwork information is accurate and that the relevant supervisors met the applicable BACB requirements.
For the monthly multiple-supervisor form, the responsible supervisor is the supervisor listed and signing the form. That signature does not erase the responsibilities of the other supervisors. It confirms that the month’s information and compliance attestation can be made by the person coordinating the arrangement.
Before the month closes, the trainee and responsible supervisor should reconcile the information that will appear on the form:
- the trainee’s name and BACB ID;
- the month and year being documented;
- the state and country where fieldwork occurred;
- the responsible supervisor’s name and certification number or BACB ID;
- independent hours, supervised hours, total hours, and percentage supervised; and
- the observation hours included in the reported month, when the form requests them.
Do not estimate from a calendar after the fact if your documentation system already contains actual hours and minutes. The safest workflow is to review the month’s records, resolve discrepancies with the supervision team, and then complete the form from the agreed information.
What goes on the monthly form?
The current organization form asks for one month of fieldwork information. It separates independent hours, when the supervisor was not present, from supervised hours, when the supervisor was present. It then records the total and the percentage of hours supervised. The form also includes an attestation that the information is true and correct and that the trainee completed fieldwork in compliance with relevant requirements and ethics requirements.
That structure is important because “I met with my supervisor” is not the same as “all of the reported fieldwork hours are documented correctly.” The monthly form is a record of the month’s fieldwork totals and supervision relationship. Keep your underlying documentation system, supervision contract, calendars, notes, and activity records consistent with the form. The form does not replace the records that support the hours.
The current form is not an invitation to copy a BACB form into a new template or redesign it for convenience. Use the official form associated with the applicable requirements, complete all required fields, and preserve the saved copy. Incomplete documents can cause the hours from the month to be lost or the form to be denied.
When must the form be signed and retained?
The current 2027 organization monthly form says it must be signed by the last day of the calendar month following the month of supervision. For example, a form documenting March supervision must be signed by the last day of April. Both the supervisor and trainee should retain a copy for at least seven years.
The form is not normally submitted to the BACB as soon as it is signed. The current form says not to submit it unless requested. That does not make the document optional. Keep the completed, signed copy in the trainee’s records so it can support an application, audit, or later review if requested.
Use a simple month-end control:
- name the file with the trainee, month, organization, and form type;
- confirm the responsible supervisor and trainee signatures are present;
- check that hours and percentage fields are complete;
- save the final PDF and reopen it to confirm that the values remain visible; and
- store the copy in a location that the trainee and supervision team can access for the required retention period.
How is the monthly form different from the final form?
The monthly form answers, “What happened during this month?” The final form answers, “What happened across the completed fieldwork period, and who provided supervision during that period?” They work together but should not be substituted for each other.
For the organization final form, the BACB FAQ says to list everyone from the organization who provided supervision. The responsible supervisor is then the person who signs the final attestation. The final form uses the monthly forms to summarize the fieldwork totals under the applicable fieldwork types. If a supervisor provided supervision during any part of the final period, leaving that person off the final form can create a mismatch between the monthly records and the final attestation.
A helpful habit is to maintain a supervisor roster from the first month. Record who provided supervision, the relevant credential or BACB ID, the dates of involvement, and whether the person was part of the same organization or a separate setting. That makes the final form a reconciliation task instead of a last-minute memory exercise.
How should you save the PDF?
BACB warns that the current fillable forms should be completed in Adobe Acrobat Reader on a desktop rather than in a web browser. The dropdown menus and date fields may not save correctly in a browser. If you prefer a paper workflow, the form instructions explain that it can be printed and completed manually over the dropdown options.
After completing the form, save it, close it, and open the saved file again. Check every page for missing dates, blank signature fields, broken dropdown values, or hours that disappeared. This readback is especially important when multiple supervisors are involved because a missing organization or responsible-supervisor field can make the document hard to reconcile later.
What mistakes can cause problems?
- Listing every supervisor on the monthly form: the current FAQ says the responsible supervisor is the person listed and signing the multiple-supervisor monthly form. The final form has the broader supervisor list.
- Combining separate organizations: use the form that matches the coordinated setting and applicable fieldwork rules; do not combine separate organizations merely for convenience.
- Using a browser to fill the PDF: fields and dates may not save correctly.
- Signing late: calendar-month deadlines are easy to miss when a month ends near a holiday or school break.
- Submitting automatically: the form should be retained and submitted only if the BACB requests it.
- Replacing the applicable form with a year-labeled template: confirm which fieldwork requirements govern the month before changing versions.
- Failing to reconcile monthly and final forms: the final supervisor list and totals should be supported by the monthly records.
For broader track study, the ABA Study Online BCBA learning path and BCaBA learning path can help organize terminology and requirements. They do not replace the current BACB handbook, official forms, supervision contract, or individualized guidance from a qualified supervisor.
Multiple-supervisor form FAQ
Do I use one monthly form for every supervisor? Not when multiple supervisors are at one organization and the organization form applies. The current BACB FAQ says the responsible supervisor is the person listed and signing the monthly multiple-supervisor form. The final form lists all supervisors from the organization who provided supervision.
What if my supervisors work at different organizations? Review the applicable coordinated-fieldwork rules and document each organization or setting with the form that matches the arrangement. Do not assume one organization form can combine every site.
When is the monthly form due? The current 2027 form says it must be signed by the last day of the calendar month following the month of supervision.
How long should I keep the form? The current form says the supervisor and trainee must each retain a copy for at least seven years.
Do I send the form to the BACB every month? No. The current form says not to submit it unless requested. Keep the signed copy available for an application, audit, or other official request.
